29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level: Sycuan Casino Blending Well: On July 15, 2022 ladders were used to adjust barb wire at the top of fencing that was 8 feet in height. A 6 Foot blue Werner A-Frame fiberglass ladder was not set up on stable level surfaces. Employee(s) were exposed to fall hazard(s).

29 CFR 1926.1053(b)(3): Ladders shall not be loaded beyond the maximum intended load for which they were built, nor beyond their manufacturer's rated capacity. Sycuan Casino Blending Well: On July 15, 2022 ladders were used to adjust barb wire at the top of fencing that was 8 feet in height. The A-frame ladder used did not have any manufacture data listed for safety requirements nor load limits for employees to identify during inspection and use of ladder. Employees were exposed to the potential of ladder overloading, mechanical failure and/or fall hazard(s).
