29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) On July 19, 2022, the employer failed to ensure that electrical equipment, such as Tripp Lite relocatable powertap mounted to the Line 19 Video Jet Machine, was used in accordance with its listing and labeling.  This powertap is intended for use with audio visual equipment and it was being used to power parts of the Video Jet Machine.  b) On July 19, 2022, the employer failed to ensure that electrical equipment, such as four outlet gang boxes, were used in accordance with its listing and labeling.  The boxes were at the end of flexible electrical cords and being used in lieu of extension cords or permanently wired outlets.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.    Visual Pak Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1910.303(b)(2), which was contained in OSHA inspection number 1319678, citation number 1, item number 2 and was affirmed as a final order on December 13, 2018, with respect to a workplace located at 3505 Birchwood Drive in Waukegan, Illinois.
