29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level: On or about July 19, 2022, an employee was exposed to a fall hazard from a scaffold from approximately 18 feet above the ground by the employer with no fall protection while conducting roofing work on a residential roof located at 6602 Guthrie Ave Cleveland, Ohio 4410.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: The employer doesn't protect employees from fall hazards while performing roofing activities on residential structures with unprotected sides and edges above 6 feet from the ground by use of guard rail system, safety net, and or personal fall arrest system. This most recently occurred on or about July 19, 2022, during the removal of roofing material at a jobsite located at 6602 Guthrie Ave Cleveland, Ohio 44102. The employer did not ensure that all employees working on the roof of a two story home were protected by a fall of approximately 20.4 feet by utilizing a fall protection system.
