(b) North side of upper roof on a single-family home; On or about July 11, 2022, fall protection was not used while employees performed roofing operations on a residential roof at heights ranging from 14 feet, 2 inches to 24 feet.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: (a) North side of a single-family home; On or about July 11, 2022, a ladder, stairway or other acceptable means of access was not provided for getting onto the upper roof from the lower roof when the elevation change was greater than 19 inches.
