Warehouse: An outside set of stairs with six  risers was not equipment with a stair rail and handrail.

29 CFR  1910.176(b):  Storage of material created a hazard. Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:   Warehouse: Products on roller shelving was not stored at a safe height to prevent boxes from sliding off the shelf. On or about 5-11-22, boxes were stacked three rows high resulting in boxes tumbling off the shelf and injuring a worker.
