29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about July 19, 2022, on the east side of the site, employees were installing an HVAC system onto the roof of a building. The employees were not in use of a fall protection system and were exposed to a fall greater than six feet to the concrete below.

29 CFR 1926.1425(b): While the operator was not moving the suspended load, no employee must be within the fall zone, except those employees engaged in; (b)(1): hooking, unhooking or guiding a load; (b)(2) initial attachment of the load to a component or structure or (b)(3) operating a concrete hopper or bucket. On or about July 19, 2022, on the east side of the site, employees installing an HVAC system and were in the fall zone of the suspended load being lifted by a crane. This exposed the employees to a struck-by hazard.
