29 CFR  1926.651(j)(2):Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:  a) Worksite was located on 4th Avenue, between the crossroads of Church Street and Valandora Court, Spring Lake, NJ 07762  Workers were exposed to caught in and struck by hazards while doing sewer lateral tie ins as they were working in an approximately five foot deep trench with an approximately three foot spoils pile on top of the north wall of the trench that was not set back or retained.  Violation was observed on or about July 20, 2022.
