29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:  On or about 14 July 2022, at Now Complex Building construction site, Estate Thomas, St. Thomas, VI.  Employees engaged on constructing wooden forms were using a portable power drill and circular saw via extension cord without GFCIs.    NOTE:  Certification of abatement is not required for this citation item since it was submitted prior to citation issuance.

On or about 14 July 2022, at Now Complex Building construction site, Estate Thomas, St. Thomas, VI.  Employees engaged on construction operations were allowed to work on top of a wood scaffold without proper fall protection system in place at a height of 13 feet high from the ground level. Employees engaged on erecting a scaffold were not supervised by a competent person as evidenced by missing proper fall protection system in place, missing cross bracing, missing base plates and missing full planking on a wood scaffold at a height of 13 feet high from the ground level.
