29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) R. Nichols Civil Contractors LLC, SE corner of E. Montview Blvd and Chester St., Aurora, CO 80010: On or about 7/20/2022, the employer's designated competent person failed to remove employees from an unprotected trench, with vertical walls, that was cut in both Type B and Type C soil with a variable depth greater than five feet. No protective systems were in the excavation. Workers were exposed to cave-in hazards.

Montview Blvd and Chester St., Aurora, CO 80010: On or about 7/20/2022, the employer did not ensure that workers were protected by a protective system while working in a trench at a depth greater than five feet cut in type B and C soils. Employees were exposed to cave-in hazards.
