29 CFR  1910.147(c)(1):  Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  Employees engaged in changing out a die block assembly on the Adamatic 10 Pocket Divider rounder machine located at 9101 Commerce Drive, De Soto, Kansas were exposed to amputation hazards.   The employer failed to lock or tag out the machine to prevent the unexpected startup of the Adamatic 10 Pocket Divider Rounder machine that caused parts of employee fingers to be amputated.

29 CFR  1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees.  Employees engaged in changing out a die block assembly on the Adamatic 10 Pocket Divider rounder machine located at 9101 Commerce Drive, De Soto, Kansas were exposed to amputation hazards.   The employer failed to provide lock out / tag out training to prevent the unexpected startup of the Adamatic 10 Pocket Divider Rounder machine that caused parts of employee fingers to be amputated.

29 CFR  1910.303(b)(2):  Listed or labeled electrical equipment was not used  in accordance with instructions included in the listing or labeling:  Employees engaged in changing out a die block assembly on the Adamatic 10 Pocket Divider rounder machine located at 9101 Commerce Drive, De Soto, Kansas were exposed to amputation hazards.   Employees were placing their hands inside the machine after the safety interlocks for the Adamatic 10 Pocket Divider Rounder machine were altered closed after raising the side panel guards to access the die block's assembly bolting screws, thus causing parts of employee fingers to be amputated.
