The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On or about July 19, 2022, at 5724 N Haymeadow Avenue in Appleton, Wisconsin, the employer did not provide required fall protection safety training for employees working at heights greater than six feet.

29 CFR 1926.1060(a)(1)(v): Stairways and ladders training requirements. The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and it shall train each employee in the procedures to be followed to minimize these hazards. The employer shall ensure that each employee has been trained by a competent person in the standards contained in this subpart. On or about July 19, 2022, at 5724 N Haymeadow Avenue in Appleton, Wisconsin, the employer did not provide required ladder safety training for employees using step ladders. Step ladders were used to access/egress a wall top plate.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a) On or about July 19, 2022, at 5724 N Haymeadow Avenue in Appleton, Wisconsin, employees were exposed to a fall hazard while working from an exterior wall top plate 16 feet above surface level without utilizing fall protection equipment. b) On or about July 19, 2022, at 5724 N Haymeadow Avenue in Appleton, Wisconsin, employees were exposed to a fall hazard while working from a second floor LVL beam 10 feet above surface level without utilizing fall protection equipment. c) On or about July 19, 2022, at 5724 N Haymeadow Avenue in Appleton, Wisconsin, employees were exposed to fall hazards while walking in a first-floor work area with four unprotected exterior wall openings without utilizing fall protection equipment. The unguarded opening created an 8 foot fall hazard with the inside edge 19.5 inches above floor level. d) On or about July 19, 2022, at 5724 N Haymeadow Avenue in Appleton, Wisconsin, employees were exposed to a fall hazard while walking outside an exterior wall in a work area with an unprotected surface level hole 9 feet above the basement surface level without utilizing fall protection equipment.
