Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  a)  On or about July 19, 2022, at 5724 N Haymeadow Avenue in Appleton, Wisconsin, employees were exposed to a fall hazard while working from an exterior wall top plate 16 feet above surface level without utilizing fall protection equipment.  b)  On or about July 19, 2022, at 5724 N Haymeadow Avenue in Appleton, Wisconsin, employees were exposed to a fall hazard while working from a second floor LVL beam 10 feet above surface level without utilizing fall protection equipment.  c)  On or about July 19, 2022, at 5724 N Haymeadow Avenue in Appleton, Wisconsin, employees were exposed to fall hazards while walking in a first-floor work area with four unprotected exterior wall openings without utilizing fall protection equipment.  The unguarded opening created an 8 foot fall hazard with the inside edge 19.5 inches above floor level.  d)  On or about July 19, 2022, at 5724 N Haymeadow Avenue in Appleton, Wisconsin, employees were exposed to a fall hazard while walking outside an exterior wall in a work area with an unprotected surface level hole 9 feet above the basement surface level without utilizing fall protection equipment.
