29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:        a) On or about July 18, 2022, employees were exposed to the hazard of falling approximately 30 feet from an upper roof deck to the ground while hoisting materials without fall protection at the roof edge.        No abatement certification or documentation required for this violation.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a) On or about July 18, 2022, employees were exposed to the hazard of falling approximately 30 feet from an upper roof deck to the ground while performing roofing work without fall protection.      b) On or about July 18, 2022, employees were exposed to the hazard of falling approximately 17 feet from an upper roof deck to a lower roof deck while performing roofing work without fall protection.      No abatement certification or documentation required for this violation.

29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators. Where manufacturer's specifications were not available, the limitations assigned to the equipment were not based on the determinations of a professional engineer competent in the field:     a) On or about July 18, 2022, roofing materials were being used as counterweighting for a hand powered hoist.   No abatement certification or documentation required for this violation.
