29 CFR 1910.106(e)(9)(iii): Combustible waste material and residues in a building or unit operating area were not kept to a minimum, stored in covered metal receptacles and disposed of daily:  Main Phillips Global building, northwest, spraying area: On or about 20 July 2022, employees did not store combustible waste material and residues in a covered metal receptacle and materials were not disposed of daily.  Combustible waste materials were stored uncovered in a metal drum after draining residuals into an enclosed drum.  Covered and uncovered, full, half-full and empty paint cans were observed next to the flammable storage locker and were allowed to accumulate.

Paint waste materials, nitrile gloves used for painting and paint saturated painting tape were located inside an uncovered metal drum that was also used for general trash.

29 CFR 1910.107(e)(2): The quantity of flammable or combustible liquids kept in the vicinity of spraying operations were not at a minimum required for operations or they exceeded a supply for one day or one shift:  Main Phillips Global building, northwest, spraying area: On or about 20 July 2022, approximately 10 marked containers of flammable and combustible liquids were observed sitting next to and across from the hazardous materials locker.  The hazardous materials locker was located on the northeast wall of the spraying area and a variety of uncovered and covered flammable liquid cans were sitting just south of the locker.  Additionally, there were more flammable liquid containers located on the west wall of the spraying area.

An employee presented a personally procured Gerson 9250 Silicone face piece size medium that was covered in dust, paint, debris and the Gerson G01 organic vapor cartridge filters had a layer of visible paint and dust.  Additionally, an employee presented an employer supplied Bullard 88VX Series Respirator Helmet Assembly w/supplied air for blasting operations also covered in dust and debris.

An employee took a personally procured Gerson 9250 Silicone face piece size medium out of his toolbox covered in dust, paint, debris and the Gerson G01 organic vapor cartridge filters had a layer of visible paint and dust.  Additionally, on 20 July 2022 and 29 September, a Bullard 88VX Series Respirator Helmet Assembly w/supplied air for blasting operations was being stored in the blasting area and another being stored in a dusty room also covered in dust.  Neither piece of equipment had a proper storage mechanism or designated area to ensure protection.
