29 CFR 1910.28(b)(13)(i): The employer did not ensure that when work is performed less than 6 feet (1.6 m) from the roof edge, each employee is protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system: a) 319 Lockheed St. Pueblo, CO 81001: On or about July 21, 2022, employer did not protect three employees from fall hazards of approximately 11 feet while performing roofing work on the roof of the first story section of a modular building. The employee's anchor was incorrectly placed on the side of the building and not on the roof itself. Employees were using harnesses and 6-foot shock absorbing lanyards with an additional length in the shock absorbing pack of 3 feet.

Three employees were using VERSICO ROOFING SYSTEMS V-150 PRIMER to apply a rubber roof which contains 60-100% toluene.
