29 CFR 1926.652(g)(2): Additional requirement for shield systems used in trench excavations. Excavations of earth material to a level not greater than 2 feet (.61 m) below the bottom of a shield shall be permitted, but only if the shield is designed to resist the forces calculated for the full depth of the trench, and there are no indications while the trench is open of a possible loss of soil from behind or below the bottom of the shield: On or about July 21, 2022, at jobsite located at 143 Armour Road, Avon Lake, OH 44012, Underground Utilities Inc. permitted its employees to enter the trench and perform work without ensuring that the bottom of the shielding system was no more than two (2) feet above the bottom of the trench. The employees were working at the bottom of a 7 feet deep trench inside of a 6 foot high X 2 foot 6 tenths wide stackable trench shield box with 2 feet and 4 tenths of unshielded soil from the bottom of the trench shield box to the trench base. Underground Utilities Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.652(g)(2), which was contained in OSHA inspection number 1392508, citation number one, item number one and was affirmed as a final order on May 14, 2019, with respect to a workplace located at 1895 Cooper Forest, Amherst, OH 44001.
