29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards while working from height during residential roofing operations. This was most recently observed on July 20, 2022 at 4534 Oak St, Omaha, NE, Nebraska when four workers were installing roofing materials to a residential structure that had an eave height of 10 feet above the ground, and the workers were not using fall protection.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:  The employer is failing to protect employees from fall hazards while using extension ladders that have been separated into two halves during roof replacement activities.  This was most recently observed at 4534 Oak St, Omaha, Nebraska on July 20, 2022 when 4 employees were using the top half of an extension ladder to access the roof of a residential structure.
