29 CFR  1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.   The employer failed to submit documentation of abatement within 10 days of the abatement date for Citation 1 Item 1 and Citation 1 Item 2.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a) On or about July 21, 2022, at the previously addressed inspection site, employees installing shingles on a residential building were exposed to falls of greater than six feet because there was no fall protection in use.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) On or about July 21, 2022, at the previously addressed inspection site, employees used a portable extension ladder to access the roof of a residential family home and the ladders  side rails did not extend three feet above the roof landing.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
