29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) For equipment at the establishment such as, but not limited to, laminators, pleaters and glue machines, on or about 07/21/2022 and at times prior to, the employer exposed employees to potential amputation and electrical shock hazards, in that, lockout tagout procedures were not available and being used for maintenance/servicing work on this equipment. Hazardous energy existed in the form of 220 VAC, compressed air, and mechanical moving parts.

29 CFR 1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) For the laminator machine, on or about 07/21/2022 and at times prior to, the employer exposed employees to electrical shock/electrocution hazards, in that, the electrical cabinet that enclosed the live, 220 VAC, electrical components for the machine was not able to be locked and was accessible to unqualified persons who were using the electrical cabinet to store personal items and a water bottle.
