A two-tier pipe scaffold that collapsed only had 3 cross braces.  On 7/20/2022 two employees were hospitalized when working a 14/16 ft. pipe scaffold that collapsed. On 7/20/2022 two employees were hospitalized when working a 14/16 ft. pipe scaffold that collapsed.  The caster wheels were observed to be in the open position on or about 07/21/2022.
