On Courtney Vista Drive in Saint Augustine, Florida: On or about July 21, 2022, the employer did not provide two underground utility construction employees a safe egress from a 13-foot excavation, in that the ladder did not reach the bottom of the excavation, and no access ramp had been constructed.

On Courtney Vista Drive in Saint Augustine, Florida: On or about July 21, 2022, the employer exposed two underground utility construction employees to a 8-foot cave-in hazard, in that the employees worked in areas outside of the trench box without a cave-in protection system.
