29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    a.  At 4310 Southside Boulevard, Jacksonville, Florida:  On or about July 22, 2022, the employer exposed employees to fall hazards ranging from 6 to 21 feet, in that fall protection, such as a body belt with a lanyard attached to the boom or basket  was not utilized, while operating an aerial lift  to repair the column on the roof of a commercial building.

29 CFR  1926.454(a):  The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a.  At 4310 Southside Boulevard, Jacksonville, Florida:  On or about July 22, 2022, the employer exposed employees to fall hazards ranging from 6 to 21 feet,  in that fall protection training  was not provided prior to these employees operating an aerial lift to repair the column on the roof of a commercial building.
