This condition  exposed employees to caught-in and pinch point hazards.  (b)Puris at 19715 231 Avenue, Harrold, SD 57536: On or about and at times prior to July 8, 2022, the employer did not ensure that employees were protected from caught-in and pinch point hazards in that installed guard, Serial Number 394, utilized on the west exterior motor of the splits plant, did not fully enclose the belt. This condition  exposed employees to caught-in and pinch point hazards.  (c)Puris at 19715 231 Avenue, Harrold, SD 57536: On or about and at times prior to July 8, 2022, the employer did not ensure that employees were protected from caught-in and pinch point hazards in that the installed yellow belt guard on the motor located at the west exterior of the splits plant did not fully enclose the belt. This condition  exposed employees to caught-in and pinch point hazards.
