29 CFR 1910.29(b)(2)(i): Midrails were not installed at a height midway between the top edge of the guardrail system and the walking-working surface: (a)Puris at 19715 231 Avenue, Harrold, SD 57536: On or about and at times prior to July 8, 2022, the employer did not sure that employees were protected from fall hazards when using the ladder/platform systems on the east side of the split plant to access elevated catwalks, in that the platforms of the systems did not include midrails. This condition exposed employees to fall hazards greater than four feet.

29 CFR 1910.219(e)(1)(i): Horizontal belt guards did not fully enclosed the belt: (a)Puris at 19715 231 Avenue, Harrold, SD 57536: On or about and at times prior to July 8, 2022, the employer did not ensure that employees were protected from caught-in and pinch point hazards in that the guarding on the Motor AP-2 belt, near the northeast corner of the split plant, did not fully enclose the belt. This condition exposed employees to caught-in and pinch point hazards. (b)Puris at 19715 231 Avenue, Harrold, SD 57536: On or about and at times prior to July 8, 2022, the employer did not ensure that employees were protected from caught-in and pinch point hazards in that installed guard, Serial Number 394, utilized on the west exterior motor of the splits plant, did not fully enclose the belt. This condition exposed employees to caught-in and pinch point hazards. (c)Puris at 19715 231 Avenue, Harrold, SD 57536: On or about and at times prior to July 8, 2022, the employer did not ensure that employees were protected from caught-in and pinch point hazards in that the installed yellow belt guard on the motor located at the west exterior of the splits plant did not fully enclose the belt. This condition exposed employees to caught-in and pinch point hazards.
