29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials, and equipment to be made by a competent person(s): (a) 2407 Stinson Ave Superior, WI, North Crude 04-H-4 Flash Heater; On or about July 15, 2022, after assembly and prior to use, the employer did not inspect Parker 7132 LP gas hose for damage, defects, and leaks. The hose connection from the vaporizer to the LP tank ruptured and a flash fire burned two employees.
