29 CFR  1926.404(b)(1)(ii):On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection.  (a) Trinidad Commercial Construction LLC at 423 Main St. Trinidad, CO 81082: On or before 7/25/22 the employer did not ensure that GFCI receptacles were grounded. GFCI and other outlets were wired with open grounds in the areas being restored. Temporary wiring strands were not grounded. This condition exposed employees to electric shock hazards. The pendant electrical cords were suspended in work areas where it could be accidentally pulled down and or damaged. This condition exposed employees to electric shock and fire hazards.
