29 CFR 1926.404(b)(1)(ii):On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection. (a) Trinidad Commercial Construction LLC at 423 Main St. Trinidad, CO 81082: On or before 7/25/22 the employer did not ensure that GFCI receptacles were grounded. GFCI and other outlets were wired with open grounds in the areas being restored. Temporary wiring strands were not grounded. This condition exposed employees to electric shock hazards. Abatement Note: Abatement Certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) Trinidad Commercial Construction LLC at 423 Main St. Trinidad, CO 81082: On or before 7/25/22 the employer did not ensure that temporary wiring was provided with strain relief in that temporary wiring was suspended from the ceiling tied to string. The pendant electrical cords were suspended in work areas where it could be accidentally pulled down and or damaged. This condition exposed employees to electric shock and fire hazards. Abatement Note: Abatement Certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."

29 CFR 1926.416(b)(2): Working spaces, walkways, and similar locations were not kept clear of cords which created a hazard to employees: (a) Trinidad Commercial Construction LLC at 423 Main St. Trinidad, CO 81082: On or before 7/25/22 the employer did not ensure that temporary wiring was distributed in a manner that kept walkways and other similar areas clear of trip hazards. Temporary wiring leading to the basement as well as pendant wires powering tools were not kept clear and away from walking and working surfaces. These conditions exposed employees to slip, trip, and fall hazards. Abatement Note: Abatement Certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."
