1910.30(b)(1): The employer did not train each employee in the proper care, inspection, storage, and use of equipment covered by this subpart before an employee used the equipment. On or about July 24, 2022, evisceration line employees engaged in cleaning/sanitation operations on the evisceration line including overhead parts. Employees utilized A-frame ladders while carrying equipment such as but not limited to water hoses and sprayer attachments without specific training on the means and methods to safely ascend and descend the ladders.

29 CFR 1910.23(c)(9): Portable ladders used on slippery surfaces were not secured and stabilized. On or about July 24, 2022, near the evisceration line A-Frame ladders are used by employees to clean the evisceration line including overhead parts on wet floors. a) a 10-foot ladder was used on a wet floor without being secured from slipping. b) a 6-foot ladder was used on a wet floor without being secured from slipping.
