On or about July 26, 2022, during the installation of roofing material for the remolding of a commercial building located at 6800 SOM Center Rd, Solon, OH 44139, the employer did not ensure employees working from a low-sloped roof were protected from a fall of approximately 24.4 feet by the use of a guardrail system, safety net system, personal fall arrest system and/or combination warning line system and safety monitoring system.
