29 CFR 1910.23(d)(2): The employer did not ensure that the minimum perpendicular distance from the centerline of the steps or rungs, or grab bars, or both, to the nearest permanent object in back of the ladder is 7 inches (18 cm), except for elevator pit ladders, which have a minimum perpendicular distance of 4.5 inches (11 cm).  a) 730 S Broad St Lansdale, PA- On or about July 26, 2022,  a fixed ladder used by employee(s) to access a pit for maintenance purposes had rungs which did exceed or equal the required 7 inch clearance from the wall which it was affixed to.

29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) Front office on 1st level on facility- On or about July 26, 2022, employees were exposed to fire and electrical hazards as extension cord was daisy chained between a relocatable power tap.  b) Treatment area on 2nd level on facility- On or about July 26, 2022, employees were exposed to fire and electrical hazards as an extension cord was daisy chained to a relocatable power tap.   For Reference:  Underwriters Laboratories (UL), refers to power strips as Relocatable Power Taps (RPTs) and, in its "General Information for Electrical Equipment Directory" (sometimes called the UL White Book or UL Directory) contains instructions that require UL-listed RPTs to be directly connected to a permanently installed branch circuit receptacle; they are not to be series-connected to other RPTs or connected to extension cords.
