29 CFR  1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and/or the fork-lift was moved horizontally while the platform was occupied:  (a) A JLG 6042 rough-terrain forklift was used to elevate workers on an uneven stack of 4' x 8' sheathing to install sheathing and other framing.  The sheathing was not a platform designed for forklift use and was not secured to the forks.  Workers were elevated approximately 13 feet above the ground.  NOTE:  Certification of corrective action (abatement) is required for this item.

29 CFR  1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l): Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178 and as referenced by the note contained in  1926.602(d).   (a) A JLG 6042 rough-terrain forklift was used on the jobsite, including to elevate workers on a stack of sheathing to install sheathing and other framing.  The operator had not received training, evaluation and certification as specified in 29 CFR 1910.178 paragraph (l).  NOTE:  Certification of corrective action (abatement) is required for this item.
