The employees used Tremco Adhesive products which contained Asphalt, Calcium Carbonate, Crystalline Silica, 1,2,4-Trimethylbenzene, and Hydrogen sulfide.

Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. a) Jobsite: 6340 Wayne Ave, Philadelphia, PA 19144 - On or about July 21, 2022, the employer failed to ensure that employees conducting roofing operations on the lower level roof of a commercial building along unprotected sides and edges, approximately 11 feet above ground level, were protected from falls by the use of a fall protection system.
