At a worksite located at 2611 Palmbrooke Way, Lot 182, in New Smyrna, Florida 32168: On or about July 26, 2022, the employer exposed an employee to head injury hazards, in that protective helmets (hard hats) were not utilized by employees working beneath other employees working from the upper roof area while installing edge dripping.

On a 6:12 pitched roof, at a worksite located at 2611 Palmbrooke Way, Lot 182 in New Smyrna, Florida 32178: On or about July 26, 2022, the employer exposed four employees to fall hazards, in that, fall protection was not utilized by employees while nailing down underlayment paper and installing drip edge. The employees were exposed to the hazard of falls ranging from 9 to 18-foot to the ground.
