(b) On or about July 19, 2022; an employee was exposed to an amputation hazard while unjamming stuck material from saw head #1 on the inline multi-head Baker band resaw machine in that its point of operation was not guarded.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed: (a) This violation occurred on or about July 29, 2022, within the top electrical box containing the circuit breakers located on the resaw line where it was found that approximately 3 inches of saw dust had settled within it exposing employees to an inhalation hazard in the event of a fire. (b) This violation occurred on or about July 29, 2022, within the bottom electrical box containing the starter switches and relays located on the resaw line where it was found that approximately 3 inches of saw dust had settled within it exposing employees to an inhalation hazard in the event of a fire. (c) This violation occurred on or about July 29, 2022, within the electrical box located on the stacking line where it was found that electrical wiring in conduit entered the box through a hole that was not effectively closed allowing saw dust to enter the box exposing employees to an inhalation hazard in the event of a fire.
