29 CFR  1910.119(f)(1)(ii)(B): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and did not address at least the steps required to correct or avoid deviation.  a) Plant; Engine Room; on or about July 23, 2022 and at times prior to,  employees were exposed to ammonia hazards, in that the employer did not ensure that compressor operation procedures addressed the consequences of Manual Operation during simultaneous controlled operation of multiple compressors, and the steps to correct or avoid deviation  to prevent an ammonia release.

29 CFR  1910.119(j)(2):The employer did not establish or implement written procedures to maintain the on-going integrity of process equipment.  a) Plant, Engine Room; on or about July 23, 2023 and at times prior to, employees were exposed to anhydrous ammonia hazards, in that the employer did not establish or implement written procedures to maintain the mechanical integrity  of compressors  2 and K and associated equipment such as process valves, relief valves, pumps, emergency shut down systems, and controls (including monitoring devices, sensors, alarms and interlocks) to prevent ammonia releases.
