29 CFR 1910.119(j)(2):The employer did not establish or implement written procedures to maintain the on-going integrity of process equipment. a) Plant, Engine Room; on or about July 23, 2023 and at times prior to, employees were exposed to anhydrous ammonia hazards, in that the employer did not establish or implement written procedures to maintain the mechanical integrity of compressors 2 and K and associated equipment such as process valves, relief valves, pumps, emergency shut down systems, and controls (including monitoring devices, sensors, alarms and interlocks) to prevent ammonia releases.
