29 CFR  1910.22(d)(3):  The employer did not ensure that when any correction or repair involves the structural integrity of the walking-working surface, a qualified person performs or supervises the correction or repair:  Roof Pit:  On, or about, July 21, 2022, an employee suffered serious injuries when the unsecured catwalk he was working from tipped and fell from the elevated work surface provided on either side of the roof pit.  The catwalk had been disconnected from the supports of the mechanical lift that had broken.  Although the instability of the unsecured catwalk was a general concern, a qualified person did not address the hazard created by the unsecured/unstable catwalk during the repair of the lifting mechanism.

Front Pit. B.  Middle Pit.

29 CFR  1910.29(b)(1): The employer did not ensure that  guardrail systems met the requirement that the top edge height of top rails, or equivalent guardrail system members, that are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface:  Front Roof Pit:  On, or about, July 21, 2022, chains used in lieu of guardrails to close off the openings in the elevated work surface when the moveable catwalk was not in position for use were measured to be less than 39 inches above the working surface.

29 CFR  1910.29(b)(2)(i):  The employer did not ensure that guardrails met the requirement that midrails that are installed at a height midway between the top edge of the guardrail system and the walking-working surface:  Roof Pit:  On, or about, July 21, 2022, the midrail provided on the moveable catwalk used at the front section was approximately 4.5 inches off center from the original design drawing and from the center of the space between the top of the toe board and the bottom of the top rail.  Employees routinely squatted and bent over to perform job duties while standing on the catwalk.
