29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   a) 39 Main Street, Belleville, NJ  Employees working from pump jack scaffolds were not protected from falls by use of a guardrail system, personal fall arrest system, or other means of fall protection. Employees worked at a height of approximately 23  feet, installing siding and insulation on a residential building.  Violation observed on 07/26/2022

29 CFR  1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.  a) 309 Main Street, Belleview, NJ  Employees worked from a pump jack scaffolds without securing the poles to a structure.  Scaffold poles were installed at an angle, were not anchored with bracing at the base, and did not have another means to prevent the base from kicking out while employees worked at heights of approximately  23 feet, installing siding and insulation on the residential building. 29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 309 Main Street, Belleview, NJ  Employees used a  Werner extension ladder to access a residential rooftop to install new insulation and siding without extending the ladder at least 3 feet above the landing surface. The access point of the roof was approximately 16-feet to the ground.  Violation observed 07/26/2022
