29 CFR 1926.502(d)(17): The attachment point of the body belt shall be located in the center of the wearer's back. The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head. On or about July 28, 2022, with respect to a worksite located at 333 Haddonstone Drive, Appleton WI 54915, the employer allowed an employee to conduct roofing work on a residential construction while their harness was attached to the lifeline at the employees front waist section of the harness.

29 CFR 1926.502(d)(21):Personal fall arrest systems shall be inspected prior to each use for wear, damage and other deterioration, and defective components shall be removed from service. On or about July 28, 2022, with respect to a worksite located at 333 Haddonstone Drive, Appleton WI 54915, the employer did not ensure that fall arrest systems were inspected for sign of wear, damage or defective components so that those components are removed from service and expose employees to potential hazards associated with falls.
