29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Employees engaged in framing activities at the jobsite located at 12508 Edward Circle, Papillion, Nebraska, were exposed to fall hazards in that side rails of a portable ladder that was used for access to an upper landing surface did not extend at least 3 feet above the upper landing surface being accessed, and the ladder was not secured at its top to a rigid support.
