29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) Porter Well Pad, 385 Willowbrook Road, Apollo, PA 15613: On or about July 25, 2022, all energy isolating devices needed to control the energy to the equipment were not operated in such a manner as to isolate the equipment from the energy source. A valve located upstream of the belly dump choke on the 2M GPU (serial number: 8141-0001) was not closed prior to replacement of the belly dump choke. An employee received serious injuries after being struck-by components of the belly dump choke that were ejected under high pressure.
