29 CFR 1926.451(e)(1): Scaffold platforms that were more than two feet (0.6m) above or below a point of access, were not provided with a means of access: On or about July 25, 2022, at a building construction site located at 7777 S. Lewis in Tulsa, OK, employees of the subcontractor were observed working on a scaffold that did not have a safe means of access, exposing the employee to the hazard of falls.
