29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about July 27, 2022, at the above addressed jobsite, employees performing residential carpentry operations, twenty-four (24) feet two (2) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  b. On or about July 27, 2022, at the above addressed jobsite, employees performing residential carpentry operations, eight (8) feet one (1) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.

29 CFR 1926.701(b): Reinforcing steel. All protruding reinforcing steel, onto and into which employees could fall, was not guarded to eliminate the hazard of impalement.    a.  On or about July 27, 2020 at the above addressed jobsite, employees working on a new house, were standing on the edge of the house, unprotected and a concrete footing was below the employees, with ends of rebar that were not guarded, thereby exposing employees to the hazards associated with impalement.

On or about July 27, 2022, at the above addressed jobsite, the portable (job-made) ladder used to access the first floor of the house, did not extend at least three (3) feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.
