29 CFR 1926.1051(a):Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) 58,59,60 Weir Drive East work site: On or about July 28, 2022, employees utilized roof rafters and stud walls to access and egress the multi-story roof work location. Employees were observed swinging from the rafters to reach the 2nd floor of the building. The elevation break exceeded 6 feet.
