Employees were replacing a second story residential roof without the use of fall protection, thereby exposing employees to fall hazards of approximately 20 feet.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee who might be exposed to fall hazards: a) Jobsite located at 1206 N Second Street, Philadelphia, PA 19122 - On or about July 27, 2022, the employer failed to provide a training program for each employee who were exposed to fall hazards. Employees were replacing a second story residential roof and were not trained on fall hazards, including how to protect themselves from such hazards, thereby exposing employees to falls of approximately 20 feet.

Employees utilized a portable ladder to access the second story roof without having side rails extended at least 3 feet above the landing surface, thereby exposing employees to fall hazards.
