29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. On or about July 28, 2022, and at times prior thereto, the employer did not establish an adequate program consisting of energy control procedures, employee training and periodic inspections where employees were performing maintenance on the Kebo Pin Setter Bowling Machine, exposing employees to caught-in hazards.

29 CFR 1910.219(f)(3): Sprocket wheels and chains that were seven (7) or less feet from the floor or platform were not enclosed: a) On or about July 28, 2022, and at times prior thereto, located at the rear of the Kebo Pin Setter Bowling Machines, employees retrieving bowling balls and releasing stuck pins were exposed to unguarded chains and sprockets driving the ball return mechanisms, exposing the employees to caught-in hazards. b) On or about July 28, 2022, and at times prior thereto, employees accessing the front and back of the Kebo Pin Setter Bowling Machines were exposed to unguarded chains and sprockets driving the pin setters, exposing the employees to caught-in hazards.
