29 CFR 1910.22(d)(1): Walking-working surfaces were not inspected, regularly and as necessary, and maintained in a safe condition:  a) Warehouse - On or about July 25, 2022, the employer did not ensure that the cargo platforms with rollers were maintained in a safe condition where cargo materials transported in the warehouse.  There were rollers missing from the platforms in various locations, creating gaps where employee's feet can get caught in.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that the strain relief was provided that will prevent pull from being directly transmitted to joints or terminal screws:  a) Warehouse Charging Station for Hydraulics - On or about July 25, 2022, the employer did not ensure that the electrical charging cord connected to the Napa Commercial Battery, charging the Shumacher Battery Charger, model #SC 1300, was connected to the device and fitting.  The charging connector had the protective sheath detached from the end and showing bare copper wire.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1910.141(a)(4)(ii): All sweepings, solid or liquid wastes, refuse, and garbage were not removed in such a manner as to avoid creating a menace to health and as often as necessary or appropriate to maintain the place of employment in a sanitary condition.  Men's Washroom - On July 25, 2022, the employer did not ensure that the men's washroom was maintained in a sanitary condition. There was garbage overflowing from the trash cans with used paper towels that were not emptied out.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
