29 CFR 1910.217(c)(2)(i):Every point of operation guard shall meet the following design, construction, application, and adjustment requirements: (a) It shall prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard; (b)It shall conform to the maximum permissible openings of Table O-10; a) On or about 07/22/2022, the employer did not ensure that the point of operation on the Alva Allen Model BT12-12 Ton Punch Press was properly guarded to prevent employees from reaching through, over, under or around the guard. b) On or about 07/28/2022, the employer did not conform to the maximum permissible openings of Table O-10. The opening measured approximately 6.26 to .750 inches (width) and 4.75 to 5.0 inches to point of operation hazard.

29 CFR 1910.217(e)(1)(i): Under the general component of the inspection program, the employer shall: (A): Conduct periodic and regular inspections of each power press to ensure that all of its parts, auxiliary equipment, and safeguards, including the clutch/brake mechanism, antirepeat feature, and single-stroke mechanism, are in a safe operating condition and adjustment (C):Maintain a certification record of each inspection, and each maintenance and repair task performed, under the general component of the inspection program that includes the date of the inspection, maintenance, or repair work, the signature of the person who performed the inspection, maintenance, or repair work, and the serial number, or other identifier, of the power press inspected, maintained, and repaired. On or before 7/22/2022 the employer did not ensure that adequate periodic inspection were being conducted on power presses such as the Alva Allen Model BT12-12 Ton Punch Press in the bond box assembly area.
