29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. a) Exterior of Building-pump jack scaffold: Employees were exposed to eye injury while engaged in siding activities without the use of eye or face protection. Employees were using pneumatic nail guns, on or about 7/29/22. b) Exterior of Building ground Ryobi table saw: An employee was exposed to eye and face injuries while using a Ryobi table saw without eye and face protection, on or about 7/29/22.

29 CFR 1926.451(b)(2)(i):Each ladder jack scaffold, top plate bracket scaffold, roof bracket scaffold, and pump jack scaffold shall be at least 12 inches (30 cm) wide. There is no minimum width requirement for boatswains' chairs. a) Building exterior left side: An employee was working from a pump jack scaffold, the scaffold platform was not at least 12 inches wide as required, on or about 7/29/22. Note to paragraph (b)(2)(i): Pursuant to an administrative stay effective November 29, 1996 and published in the Federal Register on November 25, 1996, the requirement in paragraph (b)(2)(i) that roof bracket scaffolds be at least 12 inches wide is stayed until November 25, 1997 or until rulemaking regarding the minimum width of roof bracket scaffolds has been completed, whichever is later.
