Listed, labeled, or certified equipment were not installed and used in accordance with instructions included in the listing, labeling, or certification. a). Coeur D' Alene Towneplace 1381 N. Northwood Center Court Couer D' Alene, ID. 83814 - Roof - Multiway Surge Protective Device - relocatable power tap (RPT) Model No: FS-071A3 Type 3 125V-15A 60 Hz - On or about August 1, 2022 and at times prior thereto, employees were exposed to an electrical shock of 110VAC in that the RPT was energizing two separate extension cords that were connected to DeWalt Power Drills for use to install roofing material.

29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective were used until repaired. a). Coeur D' Alene Towneplace Construction Project - 1381 N. Northwood Center Court Coeur D' Alene, ID. 83814 - Roof - On or about August 1, 2022 and at times prior thereto, employees were exposed to an electrical shock of 110VAC in that the grounding pin on the extension cord were removed/missing. The RPT and extension cord was energizing a DeWalt Power Drill for use to install roofing material.
