And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. On or about: August 1st, 2022 Location:2125 Westchester Ave, Rye, NY 10580 a) Employee #1 was engage in changing A/C filters of a unit by improperly using A-frame ladder causing him to fatally fall. Employee #1 was not provided training in the use of a stepladder and to recognize fall hazards.
