29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck: (a) On or about 07/28/2022 and at times prior to - 945 Highway 36 E, Hartselle, AL 35640 in the Warehouse, employees were exposed to struck-by hazards because 3 of 3 operators of a Doosan Model G25S and a Clark Model ESMII-15 operate the forklifts prior to completing formal training.
