Facility at 140 Centre of New England Boulevard in Coventry, RI:   a. On or about August 1, 2022, the employer did not determine if the voluntary use of a respirator created hazards including, but not limited to, cardiac and/or pulmonary disorders that could be aggravated by respirator use, wearing of a dirty respirator that can cause dermatitis or ingestion of a hazardous chemical, or sharing of a respirator that leads to transmittal of disease, for employees that are provided with or allowed to wear N-95 and elastomeric respirators.

On or about 8/1/2022, the employer did not ensure that the damaged operator's seat on the Yale powered industrial truck was replaced.  b. On or about 8/1/2022, the employer did not ensure that the parking/emergency brake on the Yale powered industrial truck was operational.  Romancing the Stone, Inc. was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.178(p)(1), which was contained in OSHA inspection number 1407387, citation number 1, item number 4c and was affirmed as a final order on 11-20-2020, with respect to a workplace located at 140 Centre of New England Boulevard, Coventry, RI 02816.
