29 CFR 1910.304(b)(3)(ii)(C)(3): Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug, was not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.   Located in the NEX back room warehouse, second floor, next to the mountain bike storage, a large HDX fan in use was missing a ground pin. A second fan, Leading Edge MDL LPM 3025, No. 1507000336, located on the first floor near the back overhead door, had frayed insulation at the connection point between the fan and the cord.  Employees were walking or working in the backroom areas where the fans were located throughout the day. Employees were exposed to an electrical hazard.
